Your First Visit

On your first visit, please bring the following:

• Your insurance card
• List of allergies
• List of current medications (prescription or over-the-counter) including:
  • Name
  • Dosage
  • Frequency
• Referral from your primary care physician, if required

Appointments

We see patients by scheduled appointment only. Scheduled appointments allow us to better serve your needs to keep waiting times at a minimum.

Please call as far in advance as possible for an appointment. If you are unable to keep it, please call to cancel so your appointment may be given to another patient.

Insurance Information

Dr. Levran participates in most all insurance plans. However, it is important to check your insurance providers list of participating physicians to be sure your program allows you to be seen at our office. Also, please review with your insurance provider the rules and limitations on coverage and reimbursement

Referrals

If you are insured by a managed care plan, it is your responsibility to obtain your referrals from your primary care physician. This pertains to all visits, including follow-ups.

Prescriptions and Refills

Please ALWAYS have your pharmacy phone number available at the time you call the office with a medical problem or refill. Medications will be refilled only during regular office hours since Dr. Levran may not have access to your medical records in the evening or on the weekend.

Fees and Billing

Charges for visits vary depending upon the services rendered.

Payment is required at the time services are rendered.

Co-payments are expected at the time of your visit.
